Gregory House Programs is seeking a dedicated and compassionate Case Manager for our Permanent Supportive Housing (PSH) initiative. This role is crucial in supporting individuals and families who have experienced chronic homelessness, with a particular focus on serving the LGBTQIA+ community. The ideal candidate will be committed to providing inclusive, culturally competent services that empower clients to maintain housing stability and improve their overall quality of life.
Responsibilities:
- Provide comprehensive, ongoing case management and supportive services to PSH participants - Conduct regular home visits and develop individualized service plans - Address barriers such as mental health, substance use, medical needs, income instability, and lack of social support - Collaborate with medical, behavioral health, and community service providers to ensure coordinated, long-term, client-centered services - Advocate for clients and support their efforts to maintain housing stability - Prioritize the unique needs of LGBTQ+ individuals and families in service delivery - Work closely with the Coordinated Entry System (CES) for client referrals - Maintain accurate and up-to-date client records and documentation - Participate in team meetings and contribute to program development and improvement
Requirements:
- Bachelor's degree in Social Work, Psychology, or related field - Minimum of 2 years experience in case management, preferably in housing or homeless services - Knowledge of harm reduction principles and housing first approaches - Familiarity with the unique challenges faced by the LGBTQIA+ community - Strong interpersonal and communication skills - Ability to work independently and as part of a team - Proficiency in Microsoft Office and case management software - Valid driver's license and reliable transportation - Bilingual skills (English/Spanish) preferred
