Behavioral Health Navigator and Tobacco Cessation Specialist Position

 

Dual role as Behavioral Health Navigator and Tobacco Cessation Specialist. Ideal candidate will be responsible for engaging with clients and families to navigate and access KKV’s full spectrum of behavioral health services in a community health center setting. This includes (but is not limited to) behavioral health counseling, substance use treatment, and recovery support, This person will also be responsible for psychosocial intervention services and development and monitoring care plan goals for patients with complex medical and psychosocial conditions.

 

Staff member will be responsible for performing behavioral health screening activities and providing education and counseling for substance use. Candidate will perform community outreach and health education activities in one-on-one and group settings. Other duties include co-facilitating recovery groups, establishing sober pro-social activities, and networking with community agencies.

 

This position is responsible for providing social service assistance support, and monitor care plan goals of patients, transportation, psychosocial and crisis support services.

 

Fifty percent of the position will be attributed to the Tobacco Prevention and Cessation Program. The hire will assist the Tobacco Program Coordinator with administrative support, data management, outreach and cessation counseling. Training is available for tobacco treatment specialist certification.

 

Bachelor's in Health and Human Services such as social work, public health, psychology, or related field required. At least one year of experience working in a community health setting is preferred. Bilingual language skills, preferred.
